Purpose of the Role:
Provide expert psychosocial advice and advanced level strategic thinking, planning and joint leadership as a member of the TPCH patient flow management team.
Provide advanced social work services and clinical leadership to the multidisciplinary teams around long stay patients where highly complex psychosocial needs are identified and could potentially impede a timely discharge.
This position provides independent clinical services across the hospital and may be required to participate in an on-call service.
Vaccine Preventable Diseases (VPD) Requirements
It is a condition of employment for this role for the employee to be, and remain, vaccinated against vaccine preventable diseases during their employment.
