About the role

An opportunity exists within our Hospital in the Home (HITH) service and within the Residential Acute Care Service (RACS) program.

The hospital substitution service, HITH, is committed to managing and supporting clients through an acute phase of illness while remaining within the familiar environment of their home or facility through the provision of client focused care.

The RACS program provides support to residential aged care facility residents by supporting facility staff via a consultative, educational and solutions focus methodology to appropriately manage residents' health needs in collaboration with General Practitioners. The principle focus is to provide the necessary acute, sub-acute and chronic care appropriately in a supportive and familiar environment. 

Salary information

The total remuneration value of up to $109,944 p.a., is comprised of salary between $89,990 - $96,361 p.a. or $45.38 - $48.60 p.h., up to 12.75% employer superannuation contribution and 17.5% leave loading, Nurse Grade 6 (1). 

Vaccine Preventable Diseases (VPD)

It is a condition of employment for this role for the employee to be, and remain, vaccinated against vaccine preventable diseases as listed on the Role Description.
