About the Role
Provide direct follow up, support and liaison with and on behalf of Aboriginal and Torres Strait Islander patients and their families by accessing resources and services within The Prince Charles Hospital.
Provide Aboriginal and Torres Strait Islander patients, escort and/or family members with a coordinated service approach to hospital admissions and discharge.
Facilitate and maintain liaison and cultural support links with staff to enhance access for Aboriginal and Torres Strait Islander patients and their families when presenting to health services.
Provide practical social support services to Aboriginal and Torres Strait Islander patients, escort and their families during their episode of care. This includes an advocacy role and developing linkages with all relevant hospital wards, departments and community services to facilitate admission, discharge planning and referral to follow-up services.
Network and liaise between patients, their families, Aboriginal and Torres Strait Islander community organisations, staff of the Metro North Hospital and Health Services; Indigenous Primary Care teams, Health partner organisations (such as GP and Community Controlled sector) and other government sector agencies to promote patient access to appropriate health and social services.
Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
