Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing
To work independently and collaboratively in a multidisciplinary team utilising professional skills and ability to contribute to client centred care to meet the client’s individual health care needs, with the aim of maintaining and/or improving their health
To provide nursing assessment, care coordination, evidence based best practice clinical intervention, monitoring, maintenance and education to clients and their carers/families

- Ability to work as a member of a multidisciplinary team
- Commitment to continuing professional development
- Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training
- Current Drivers Licence valid within Australia and willingness to use for work purposes
