Blackpool Teaching Hospitals NHS Foundation Trust has a centralised Patient Flow Team, which covers all Acute and Community in-patient beds across two Hospital sites. The Trust's aim is to ensure patients get the right care, at the right time in the right place. The Patient Flow Team is central to supporting this as it oversees both the admission and discharge of in-patient care, ensuring that patients are offered safe alternatives to admission if possible, and ensuring patients ready for discharge are helped to return home in a safe and appropriate way. The Patient Flow Team comprises of Clinical Flow Matrons who support efficient, effective and safe management of beds throughout the hospital sites during ‘core hours’ and the ‘out of hours’ period and provide clinical and managerial leadership ensuring a safe environment with optimum patient care. The team also includes Patient Flow Facilitators who support the co-ordination of beds throughout the Trust support to optimise patient care and safety.
You will provide strong leadership and clinical expertise to ensure effective and efficient use of resources across the Trust. The successful candidate will assume the responsibility for site cover ‘out of hours’ and ensure appropriate communication with the senior on-call manager.
We are seeking 2 highly motivated, experienced Nurses with strong leadership skills and substantial clinical/management experience in acute and community health services including patient flow. Candidates should have the commitment and passion to inspire and encourage clinical staff across the Trust to continue to deliver high standards of care.
