About the opportunity

An exciting opportunity exists for a Registered Nurse to join our Integrated Care team in Western NSW LHD.  This part time position based in Forbes will  report to the Manager of Integrated Chronic and Complex Care . The Integrated Care Coordinator will work collaboratively with clients, carers , Gps and staff across the Community, Social and Health Care sectors to deliver  effective Integrated Care Interventions for people living with chronic health conditions and complex care needs.

What you'll be doing
The Integrated Care Coordinator is an integral part of the multidisciplinary team providing a holistic assessment and delivering care coordination, care navigation and health coaching for people with Chronic  Health Conditions and  Complex care needs who are registered in Western NSW LHD  Integrated Care program.

  1. Current Authority to Practise as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  2. Demonstrated ability to practice in accordance with the Registered Nurse Standards of Practice, Code of Ethics for Nurses, and Code of Professional Conduct for Nurses (Nursing and Midwifery Board of Australia).
  3. Demonstrated effective clinical communication through the use of information technology, written and verbal means; and sound interpersonal skills including the ability to communicate with a diverse range of people.
  4. An understanding of patient centred care and demonstrated ability to work effectively both independently and as part of the multi-disciplinary health care team.
  5. Recent clinical experience relevant to the position and commitment to quality improvement, practice development and evidence-based practice.
  6. Demonstrated ability to lead, supervise, delegate, and negotiate with others to deliver quality nursing care.
  7. Demonstrated ability to effectively prioritise and organise workload and deliver care with integrity and accountability.
  8. Demonstrated experience providing primary care in a rural setting using a person centred holistic  approach to develop and support implementation of individualised care plans
