The Extensive Care service formed as part of the New Models of Care program, which was launched in 2015.  The Extensive Care Service delivers clinics across 5 sites on the Fylde Coast, which all GP practices, Neighbourhood Care Teams and Secondary Care services can refer into. The service is a Community based service providing coordinated and integrated care for patients over the age of 18 with at least one long term condition including frailty and dementia.

The team is multidisciplinary team consisting of medical staff, Advanced Practitioners, Trainee Advanced Practitioners, Community Matrons, Clinical Care Coordinators, Pharmacists, Pharmacy Technicians and Assistant Practitioners.

You will undertake a generic role as a Clinical Care Coordinator learning skills and knowledge from other professionals to provide an integrated model of care to patients aiming to ensure their care is provided in a coordinated way but also supporting them to manage their own care better. We are looking for the successful candidate to be able to:

• Assess, plan implement and evaluate care.
• Ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team.
• Work in partnership with the wider community and primary care teams to provide holistic patient care.
• Be able to demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients, carers and other professionals.

This post holder will need to have good communication skills both written and verbal, have evidence of ongoing professional development and be able to work independently.

The post holder will either be based from South Shore Primary Care Centre or Moor Park Health Centre, but will be expected to travel across the Fylde Coast region to effectively case manage and support with clinics.  Applicants must have a full driving license and access to a vehicle
