Newcastle Diabetes Service is located at the Newcastle Community Health Centre
in Newcastle West. Education services are also provided for patients with
diabetes (type 1, 2 and gestational), their carers’ and support people, across
multiple sites in the community, including the John Hunter Outpatient
is dynamic and responsive to the changing needs of the patients within our
service area. Patients, carers, support people are provided with individualised
excellent patient care within a community health environment to maximise the
health outcomes for the patient. The service also provides education to health
care professionals and external service providers who care for people with

the opportunity to be a part of multi-disciplinary team to provide assessment
and individual and group education to assist patients to improve and
self-manage their diabetes, and to provide consultation and information to
other clinicians, as required, to facilitate the management and care of
patients with diabetes. This is achieved through activities directed by the
Service Manager, GNS Diabetes, and through tasks identified by the Diabetes
Clinical Stream and the Hunter Alliance Diabetes Workstream.
The Greater Newcastle Sector Diabetes Service is looking
for an experienced registered nurse with enthusiasm, who has excellent skills
in managing patients with diabetes, good written and verbal communication
skills, the ability to provide clinical support to internal and external staff
through mentoring and education presentations and a comprehensive knowledge of
current diabetes management strategies.
