We are seeking a highly motivated individual to work within our 24 hour Unscheduled Care Team. The team provides advanced clinical care in a variety of settings, including residential homes, patients’ homes and other community settings as required.

The post holder MUST be a registered nurse or a paramedic and hold a relevant Advanced Practitioner qualification.

We would consider applications from candidates who do not meet all the essential criteria but would be interested in a band 6 training post.

The key aims of the service are to facilitate early discharge from hospital and to reduce unnecessary A&E attendance. The team also aim to reduce the demand for hospital admissions by providing the right care at the point of need. This exciting and challenging role offers the post holder the opportunity to provide high quality patient focused care in the community, providing clinical care, including telephone triage within the Unscheduled Care Team and if necessary within the Single Point of Access Team, diagnosis, treatment, referral, review or discharge as appropriate. The post holder will hold and utilise a range of advanced skills and have expert knowledge to assess patients with a range of undifferentiated and undiagnosed conditions. This service currently operates seven days a week, 24 hours a day.

You must also be able to demonstrate experience of working autonomously and possess excellent communication skills. You will be required to work effectively and autonomously within the team and across the multidisciplinary sector. Community experience is desirable, however appropriate training and development will be offered to those with a wide range of transferable skills and experience.
