Band 7 Occupational therapy Co-ordinator for Heart Failure Patients

Permanent Full time post 37.5 hours per week

A vacancy has arisen for a band 7 Health Professional to take a lead role in the advanced assessment and treatment planning of heart failure patients requiring co-ordinated rehabilitation management.

You will be expected to ensure patients receive an enhanced rehabilitation process for the entire patient’s journey through hospital to follow up in the acute hospital wards and community, ensuring continuity of care and seamless transitions between care providers beyond the acute hospital environment.

You will need to undertake a team leader role alongside other Clinical Leads and the Hospital Physiotherapy and Occupational Therapy Manager, in planning, co-ordinating, delivering and evaluating rehabilitation services on a day to day basis.

You will have responsibility for the continuing professional development and overall performance of Band 6 and band 5 Staff, Students and Assistants.
