Clifton Hospital

We are expanding our Advanced Clinical Practitioner (ACPs) workforce at Clifton Hospital to support the development of our Nurse/Therapy Led model of care. Clifton Hospital is a great place to work with dedicate staff and a real sense of community, so come join us.

The ACP role will from December 2019 cover the hours between 8am and 8pm seven days a week and therefore we can offer full and part time working over a variety of shift patterns. We are looking to appoint staff that can cover a mixture of days/hours to cover this period across the week.

We are looking for experienced ACP's (qualified to Masters level including NMP) to join us. Clifton Hospital provides rehabilitation to patients embracing a MDT approach. We continually work to improving the quality of care we provide to patients and ACPs are key to this.

The majority of our patients are currently transferred to Clifton Hospital following an acute episode of care at Blackpool Victoria Hospital and are deemed as medical fit for discharge to Clifton Hospital for a period of rehabilitation. We also accept patients that are admitted to Clifton Hospital direct from the community (step-up) from our community teams and in time direct from GP’s/primary care.  

You will be supported in your role by an onsite Nurse Consultant and other ACP colleagues as well as the onsite Clinical Matron. Regular clinical supervision and educational sessions are provided on site.
